Behavior Incidents are highly confidential sensitive data and should be handled securely.
Behavior student data access is by permissions only.
The behavior application tracks conduct of students. Incidents can be both positive and negative. Incidents can be created, viewed, and submitted to a school administrator or a behavior tracker as a referral by a teacher. Administrators can create and review all incidents and referrals created by faculty and staff.
School Administrators over behavior can take action on any incident or referral.
Teachers who submit an incident for administrators to follow up with as a referral, only see what they have put into the information.
Teachers only see their own students who are scheduled in their courses. Teachers do not see other teacher’s information about students. Teachers do not see what a school administrator has added or edited on the behavior incident the teacher has submitted.

Recording an Incident
Select -Record Incident –
New Incident -
Add - Offender – by selecting + sign
Add – Victim, if there is a victim (optional) – by selecting + sign
Add - Both Offender-Victim when a student is both (optional) – by selecting + sign
Add - Witness if there is a witness –(optional) – by selecting + sign
Student Search will come up - type in the name or the student ID #
Select the student(s)
Selected Student will be added to the Role as the Offender, Victim or Both Depending on what you selected.

This information is not submitted to the State -
Everything in the Blue Box below needs to be filled out by the school admin. This data is what is sent to the State via UTREx submissions.
Student Involvement - Only select infractions that are applicable.
If Gang Related, Injured, or Weapon was not involved, do not select
Emergency Interventions Used: Was Physically Restrained, Was Placed in Seclusion was not done, do not select.
Alleged Basis of Incident - Only select infractions that are applicable.

Managing Behavior Incidents – Submitted by Teachers
When an incident has been referred by a teacher – It will display on the School Lens
School Lens -
Select the Behavior Incident for viewing –
A behavior tracker or school administrator will add to or modify the incident submitted by the teacher – finish necessary data and Save Incident.
Leaving the Viewed by Administration checked means - will not need any further action to this incident.
Add Description notes for critical information that is necessary to know what happened.
Putting in good information is critical for accounting the incident at later dates. Law enforcement may request incident information that let up to a severe incident.
Searching Behavior Incidents –
Incidents can be searched by Incident Number, Grade Level, Behavior Type, Student Name
Reported By, only includes users who have entered incidents.
Adding Locations for behavior incidents
To add locations - go to the Lockers Tile on the School Lens - select Locker Attributes.
Locations are shared with Lockers and Behavior, so you only must set them up once for both.
