10 STEPS
1. There are two ways you can find the Public Data Request form to fill out.
2. The first step is too open apphelpdesk.schools.utah.gov
Click Create Account / Login
3. A user has 3 options when creating their account, utilize a Microsoft or Google account or you can create your own login and password.
For training purposes, we will click Sign in with Google
4. Click the account you want to login with.
5. If you need something, such as to request a new account, a password reset or another service item.
Click Request a service
6. Or the user can type public in the search bar.
7. It will start showing service items for you to select, select the one that you are looking for.
8. Type public in the search bar, it will start showing service items for you to select, select the one that you are looking for.
9. Scroll down and fill out the form. Information that needs to be filled in will have a Red asteric.
10. Click Place Request
