How to Submit SSID Merge Requests

Thank you for contacting the USBE Help Desk. We'll be happy to provide you with the navigation steps to merge these two SSIDs together.

A couple of things are required to ensure that the merge is executed smoothly.

1. ALL information of the SSIDs being merged needs to be identical. In this case, I cannot merge the two SSIDs since they have different Gender Indicators. Check the documentation on file for the student, and correct the information if it's on your end. (Hint: it looks like the clerical error is on the end of American Preparatory Academy, so they'll need to work with you to verify with the parents/documentation to confirm the student's legal name, DOB, and Gender ID. Once it's been verified that both students have the same parents, and matching PII, then you're good! If the clerical error is on your end, then you can simply go into the "Update Student Info" under Student Maintenance section of the SSID system.)

2. Other LEAs (Local Education Agencies) tied to the SSID number being "merged away" need to be informed and agree to the merge. They verify parent/guardian and student details to help determine which record is accurate. Since we are modifying state SSID information. In addition, there's a possibility that other LEAs need to be given the opportunity to prevent loss of data of the previously enrolled student if the LEA was using the higher SSID Number. To find out the details of which other LEAs the student has been at in Utah, click the "history" button for both SSIDs. From here, you'll see on the right hand side there's a list showing historically where the student has been attending school (for the two SSIDs in this case, you'll need to reach out to: 74-American Preparatory Academy (since American Prep-Salem is no longer an active LEA) & 19-Nebo District).

Once you reach out, and receive confirmation from the other LEAs on the clerical discrepancy (if any) you'll be able to submit the SSID Merge Request! Below are some helpful screenshots highlighting the section of the SSID system you'll utilize.

Here's the how to navigate the website to submit the SSID Merge Request. Start at the Help Desk System Home page: https://apphelpdesk.schools.utah.gov/support/home

Once you pull up the Service Catalog, you'll be able to navigate to the SSID Merge requests:

You'll be prompted to submit all pertinent information on this form:

Reminder: The lower of the two SSID Numbers will be kept. the requesting LEA needs to provide the student school id number to keep, and provide other LEA contacts & LEA# confirming permission was granted.

This will generate a service request ticket in Fresh Services, and you'll be notified if there's an issue, and also when the merge is complete.
